I think medium and low have V-Sync off, if you really want to be sure it's off, you'd have to set the graphics to medium or low until they finally patch those problems. What you could try is going into the advanced options, turning V-Sync off, save in advanced options, go back, when you're back at the basic options window, DON'T save when prompted again
